In addition to a completed application or resume, we will need the following documents:

• Current Driver’s License or ID
• Social Security Card
• Current TB Skin Test
• Current Criminal Background Check (Please visit your local Sheriff’s Department for fingerprinting. We will need 2 copies of your fingerprint cards.)

Prior to assignment, we’ll meet with you to complete all orientation forms and review agency policies. Documents can be uploaded or delivered in person. Please be sure to contact our office to schedule a time if you plan to drop off any documents or paperwork.
